Hypnotherapy Waiver & Terms & Conditions

1. Acknowledgment of Hypnotherapy

I understand that hypnotherapy is a self-regulated practice and that it is designed to

help facilitate positive changes in behaviour and emotions. I acknowledge that the

hypnotherapist is not a medical doctor or licensed mental health professional.

2. Voluntary Participation

I agree to voluntarily participate in hypnotherapy sessions. I understand that I may

refuse any suggestions or techniques provided during the session and may stop the

session at any time if I feel uncomfortable.

3. Informed Consent

I have been informed of the nature of hypnotherapy and understand that while many

people benefit from hypnotherapy, no specific outcomes or results are guaranteed. I

also understand that the success of the sessions depends on my willingness and

active participation.

4. Medical Disclaimer

I acknowledge that hypnotherapy is not a replacement for medical treatment, mental

health care, or medication. I will consult with my doctor or healthcare provider if I

have any concerns about my health before participating in hypnotherapy.

5. Liability Waiver

I hereby release and hold harmless Jason Tong of Fix Me Audios & Hypnotherapy, and any

affiliated entities from any claims, liability, or damages arising from my participation

in hypnotherapy sessions. I understand that I am fully responsible for my mental,

emotional, and physical well-being during and after the hypnotherapy sessions.

6. Cancellation Policy

I understand that in the event I must cancel or postpone a session, a minimum of 12

hours notice must be given, to avoid full or part session fee being charged at the

discretion of Jason Tong. In the event that Jason Tong has to reschedule, no

compensation will be made. Due to the preparation work involved and/or the time

slot reserved, no refunds will be made.

7. Results may vary
Results from hypnotherapy sessions, products, or services may vary from person to person. No guarantees or promises are made regarding specific outcomes.
